Denna studie visar att HOPE, en personcentrerad intervention via skolsk&#x00F6;terskor i elevh&#x00E4;lsan gav sm&#x00E5; men signifikanta minskningar av ungdomars problem med insomni och f&#x00F6;rb&#x00E4;ttring av deras upplevda h&#x00E4;lsa. HOPE &#x00E4;r ett arbetss&#x00E4;tt som skolsk&#x00F6;terskor kan anv&#x00E4;nda f&#x00F6;r att hj&#x00E4;lpa ungdomar att hantera sm&#x00E4;rta och s&#x00F6;mn p&#x00E5; ett b&#x00E4;ttre s&#x00E4;tt.</p><sec id="sec1"><title>Introduktion</title><p>M&#x00E5;nga ungdomar lider av l&#x00E5;ngvarig sm&#x00E4;rta och insomni, vilket kan p&#x00E5;verka deras h&#x00E4;lsa och v&#x00E4;lbefinnande (<xref ref-type="bibr" rid="R4">Finan et al., 2013</xref>; <xref ref-type="bibr" rid="R8">Haraldstad et al., 2011</xref>). I denna studie definierades ungdomar som personer i &#x00E5;ldersgruppen 13&#x2013;19 &#x00E5;r och l&#x00E5;ngvarig sm&#x00E4;rta som ett tillst&#x00E5;nd som varat i minst tre m&#x00E5;nader. Insomni inneb&#x00E4;r att personen sov f&#x00F6;r lite eller f&#x00F6;r d&#x00E5;ligt under en l&#x00E4;ngre tid, och hade problem med att somna, sova ost&#x00F6;rt eller vakna f&#x00F6;r tidigt (<xref ref-type="bibr" rid="R14">World Health Organization, 1992</xref>). Det har i tidigare forskning p&#x00E5;visats att det ofta finns ett &#x00F6;msesidigt samband mellan l&#x00E5;ngvarig sm&#x00E4;rta och s&#x00F6;mnproblem (<xref ref-type="bibr" rid="R4">Finan et al., 2013</xref>; <xref ref-type="bibr" rid="R5">Fishbain et al., 2010</xref>), vilket inneb&#x00E4;r att sm&#x00E4;rta kan f&#x00F6;rs&#x00E4;mra s&#x00F6;mnen, men ocks&#x00E5; att d&#x00E5;lig s&#x00F6;mn kan f&#x00F6;rv&#x00E4;rra sm&#x00E4;rtupplevelsen. Det tyder p&#x00E5; att effektiv sm&#x00E4;rtlindring kan beh&#x00F6;va ta h&#x00E4;nsyn till b&#x00E5;de sm&#x00E4;rta och s&#x00F6;mnproblem (<xref ref-type="bibr" rid="R2">Bjurstrom &#x0026; Irwin, 2016</xref>; <xref ref-type="bibr" rid="R4">Finan et al., 2013</xref>). Ungdomar beh&#x00F6;ver &#x00E5;tta till tio timmars s&#x00F6;mn per natt f&#x00F6;r att m&#x00E5; bra, och f&#x00F6;r lite s&#x00F6;mn kan leda till p&#x00E5;verkan p&#x00E5; h&#x00E4;lsa (<xref ref-type="bibr" rid="R9">Hirshkowitz et al., 2015</xref>). S&#x00F6;mnbrist under en l&#x00E4;ngre tid kan f&#x00E5; konsekvenser bland annat i form av &#x00F6;kad irritabilitet, nedsatt kognitiv funktion (<xref ref-type="bibr" rid="R1">Anderson et al., 2009</xref>) och koncentration, beslutsfattande och motivation, samt &#x00F6;kad risk f&#x00F6;r &#x00E5;ngest och depression (<xref ref-type="bibr" rid="R3">Carskadon, 2011</xref>). Besv&#x00E4;ren kan g&#x00F6;ra det sv&#x00E5;rt att klara av att g&#x00E5; i skolan, umg&#x00E5;s med kompisar och delta i fritidsaktiviteter (<xref ref-type="bibr" rid="R8">Haraldstad et al., 2011</xref>). D&#x00E4;rf&#x00F6;r &#x00E4;r det viktigt att erbjuda tidiga insatser som kan hj&#x00E4;lpa ungdomarna att hantera dessa problem.</p></sec><sec id="sec2"><title>Syfte</title><p>I den h&#x00E4;r studien (<xref ref-type="bibr" rid="R12">Wallbing et al. 2023</xref>) utv&#x00E4;rderade forskarna effekterna av en intervention som kallas Help Overcoming Pain Early (HOPE), som erbj&#x00F6;ds av skolsk&#x00F6;terskor till ungdomar med l&#x00E5;ngvarig sm&#x00E4;rta i skolmilj&#x00F6;n via elevh&#x00E4;lsan (<xref ref-type="bibr" rid="R7">Nilsson et al., 2019</xref>).</p></sec><sec id="sec3"><title>Metod</title><p>HOPE beskrivs mer utf&#x00F6;rligt i tidigare artiklar (<xref ref-type="bibr" rid="R6">Fors et al., 2020</xref>; <xref ref-type="bibr" rid="R7">Golsater et al., 2019</xref>; <xref ref-type="bibr" rid="R11">Rosvall, 2020</xref>; <xref ref-type="bibr" rid="R13">Wigert et al., 2021</xref>) och kan sammanfattas enligt nedan. HOPE bestod av fyra individuella tr&#x00E4;ffar med skolsk&#x00F6;terskan och ungdomen. Tr&#x00E4;ffarna bestod av tv&#x00E5; huvuddelar. F&#x00F6;r det f&#x00F6;rsta ingick utformningen av en personcentrerad h&#x00E4;lsoplan d&#x00E4;r individuella m&#x00E5;l sattes. F&#x00F6;r det andra ingick en utbildningsdel som syftade till att f&#x00F6;rklara sambandet mellan stress, sm&#x00E4;rta och relaterade problem utifr&#x00E5;n ett biopsykosocialt perspektiv. Den h&#x00E4;r delen inneh&#x00F6;ll ocks&#x00E5; personligt anpassade s&#x00E4;tt att hantera sm&#x00E4;rta, stress och s&#x00F6;mnbesv&#x00E4;r och inkluderade strategier f&#x00F6;r att uppn&#x00E5; struktur, aktivitetsbalans och &#x00E5;terh&#x00E4;mtning, genom fysisk aktivitet och avslappnings&#x00F6;vningar. Skolsk&#x00F6;terskans utbildningsmaterial om s&#x00F6;mn inneh&#x00F6;ll specifika omr&#x00E5;den som togs upp i samtal med ungdomar n&#x00E4;r de diskuterade s&#x00F6;mnproblem. Dessa inkluderade att identifiera faktorer som underl&#x00E4;ttar s&#x00F6;mn, som l&#x00E4;mplig rumstemperatur, ett tillr&#x00E4;ckligt tyst och m&#x00F6;rklagt rum och att minimera sk&#x00E4;rmtid och smartphoneaktivitet i samband med l&#x00E4;ggdags. Dessutom gavs information om hur en tupplur, fysisk aktivitet, matintag och vissa mediciner p&#x00E5;verkar s&#x00F6;mnen. Slutligen fr&#x00E4;mjades strategier som fysisk aktivitet och avsp&#x00E4;nningstekniker f&#x00F6;r att effektivt hantera kv&#x00E4;lls- och nattstress, oro och &#x00E5;ngest.</p><p>Forskarna anv&#x00E4;nde sig av data fr&#x00E5;n en tidigare studie i vilken HOPE j&#x00E4;mf&#x00F6;rdes med en v&#x00E4;ntelista-kontrollgrupp. Det prim&#x00E4;ra utfallsm&#x00E5;ttet i den studien var tillit till sin egenf&#x00F6;rm&#x00E5;ga i dagliga aktiviteter (self-efficacy in daily activities) (<xref ref-type="bibr" rid="R6">Fors et al., 2020</xref>). I den aktuella studien som redovisas h&#x00E4;r inkluderades alla ungdomar som hade f&#x00E5;tt HOPE-interventionen, &#x00E4;ven de som f&#x00F6;rst hade varit i v&#x00E4;ntelista-kontrollgruppen.</p><p>Insomni och sj&#x00E4;lvskattad h&#x00E4;lsa m&#x00E4;ttes med fr&#x00E5;geformul&#x00E4;r innan och efter behandlingsprogrammet och vid en uppf&#x00F6;ljning efter sex m&#x00E5;nader. Minimal Insomnia Symptom Scale (MISS) anv&#x00E4;ndes f&#x00F6;r att bed&#x00F6;ma symtom p&#x00E5; s&#x00F6;mnl&#x00F6;shet, och sj&#x00E4;lvskattad h&#x00E4;lsa (SRH) utv&#x00E4;rderades med hj&#x00E4;lp av en enskild fr&#x00E5;ga. Andra variabler som m&#x00E4;ttes innan behandlingen p&#x00E5;b&#x00F6;rjades var &#x00E5;lder, k&#x00F6;n, sm&#x00E4;rtlokalisation, sm&#x00E4;rtp&#x00E5;verkan, skolfr&#x00E5;nvaro och depressiva symtom (m&#x00E4;tta med Center for Epidemiological Studies Depression Scale for Children) (<xref ref-type="bibr" rid="R7">Nilsson et al., 2019</xref>).</p></sec><sec id="sec4"><title>Resultat</title><p>Resultaten indikerade att ungdomarna som deltog i HOPE hade sm&#x00E5; men signifikanta f&#x00F6;rb&#x00E4;ttringar av sina symtom p&#x00E5; insomni vid sexm&#x00E5;nadersuppf&#x00F6;ljningen, och sm&#x00E5; men signifikanta f&#x00F6;rb&#x00E4;ttringar av sin sj&#x00E4;lvskattade h&#x00E4;lsa b&#x00E5;de efter programmet och vid sexm&#x00E5;nadersuppf&#x00F6;ljningen.</p></sec><sec id="sec5"><title>Etiska och metodologiska &#x00F6;verv&#x00E4;ganden</title><p>Studien f&#x00F6;ljde etiska riktlinjer enligt Helsingforsdeklarationen (<xref ref-type="bibr" rid="R15">World Medical Association, 2013</xref>) och godk&#x00E4;ndes av Etikpr&#x00F6;vningsn&#x00E4;mnden i G&#x00F6;teborg (Diarienummer 172-16). Alla deltagare i studien, eller n&#x00E4;rmast anh&#x00F6;riga, fick information och gav sitt skriftliga samtycke till att delta.</p><p>Forskarna p&#x00E5;pekar att studien har flera begr&#x00E4;nsningar, till exempel att den inte n&#x00E5;gon j&#x00E4;mf&#x00F6;relsegrupp och att det inte g&#x00E5;r att utesluta att andra faktorer &#x00E4;n HOPE kan ha bidragit till f&#x00F6;r&#x00E4;ndringarna. Samtidigt &#x00E4;r detta en av f&#x00E5; studier som har f&#x00F6;ljt upp ungdomar under sex m&#x00E5;nader och med flera m&#x00E4;tpunkter. D&#x00E4;rf&#x00F6;r beh&#x00F6;vs mer forskning f&#x00F6;r att fastst&#x00E4;lla HOPE:s effektivitet och f&#x00F6;rklara hur och f&#x00F6;r vilka programmet fungerar b&#x00E4;st.</p></sec><sec id="sec6"><title>Slutsats</title><p>Sammanfattningsvis visar studien att det finns sk&#x00E4;l att vidare utv&#x00E4;rdera HOPE i framtida studier, och att det kan vara ett anv&#x00E4;ndbart program f&#x00F6;r att ge tidig hj&#x00E4;lp till ungdomar med l&#x00E5;ngvarig sm&#x00E4;rta och symtom p&#x00E5; insomni i skolmilj&#x00F6;n, innan de beh&#x00F6;ver ut&#x00F6;kade insatser. Genom en personcentrerad arbetsmodell kan skolsk&#x00F6;terskan m&#x00F6;ta ungdomarna och ge dem f&#x00F6;ruts&#x00E4;ttningar att f&#x00F6;r&#x00E4;ndra sin situation.
