<title>Abstract</title>
<p>Background: Difficulty sleeping is a common problem in the elderly, which has a negative impact on activities in daily life and leads to psychological and social limitations. To improve sleep, the elderly need to perform meaningful activities. Despite this, leisure activities are often overlooked. Purpose: The purpose of the study was to describe the effect of leisure activity in improving sleep in the elderly. Method: The study was conducted as a systematic literature review. Results: The included 18 studies were of randomized controlled design (RCT). The age of the study participants varied between 65 and 97 years. The included studies describe which leisure activities have had an effect on sleep in the elderly: Tai Chi/ Hatha Yoga/ Pilates (n=6); Walking/ Strength training/ Home gym (n=5); Housework/ Social group activity and physical activity (n=2); Water aerobics/ Hand massage (n=2); Dancing/ stretching/ computer games (n=3). Conclusion: Involvement in leisure activity could be a non- pharmacological health promoting intervention for elderly people with sleep problems. Therefore, leisure activity should be prioritized more in the health promoting work when treating sleep problems.</p>

<title>Inledning</title>
<p>S&#x00F6;mnsv&#x00E5;righeter &#x00E4;r ett vanligt f&#x00F6;rekommande h&#x00E4;lsoproblem hos &#x00E4;ldre vilket kan leda till psykiska och sociala begr&#x00E4;nsningar i det dagliga livet. Syftet med den systematiska litteratur&#x00F6;versikten var att beskriva effekten av fritidsaktiviteter f&#x00F6;r att f&#x00F6;rb&#x00E4;ttra s&#x00F6;mn hos &#x00E4;ldre. Resultatet visade att fritidsaktiviteter skulle kunna vara en icke-farmakologisk h&#x00E4;lsofr&#x00E4;mjande &#x00E5;tg&#x00E4;rd som borde prioriteras i behandlingen av s&#x00F6;mnproblem hos &#x00E4;ldre.</p>

<title>Bakgrund</title>
<p>F&#x00F6;rekomsten av s&#x00F6;mnproblem &#x00F6;kar med &#x00E5;ldern bl.a. p&#x00E5; grund av f&#x00F6;r&#x00E4;ndringar i dygnsrytmen. S&#x00F6;mnproblem hos &#x00E4;ldre &#x00E4;r idag ett globalt h&#x00E4;lsoproblem (WHO, 2015). Bristande s&#x00F6;mn &#x00E4;r f&#x00F6;rknippad med en &#x00F6;kad risk f&#x00F6;r olika sjukdomar och kan &#x00E4;ven leda till fysiska och sociala begr&#x00E4;nsningar i det dagliga livet. F&#x00F6;rskrivning av l&#x00E4;kemedel mot s&#x00F6;mnbesv&#x00E4;r har &#x00F6;kat i Sverige de senaste 25 &#x00E5;ren (<xref ref-type="bibr" rid="R63">Socialstyrelsen, 2017</xref>; <xref ref-type="bibr" rid="R57">SBU, 2010</xref>). Farmakologisk behandling mot s&#x00F6;mnbesv&#x00E4;r kan leda till &#x00F6;kad fallrisk hos &#x00E4;ldre men har &#x00E4;ven en negativ p&#x00E5;verkan p&#x00E5; milj&#x00F6;n. Det &#x00E4;r d&#x00E4;rf&#x00F6;r viktigt att beakta icke-farmakologisk behandling vid s&#x00F6;mnbesv&#x00E4;r (<xref ref-type="bibr" rid="R53">Richards et al., 2005</xref>; <xref ref-type="bibr" rid="R75">Vogel et al., 2021</xref>). Forskning visar att fysisk aktivitet kan vara viktigt f&#x00F6;r att f&#x00F6;rebygga s&#x00F6;mnbesv&#x00E4;r hos &#x00E4;ldre (<xref ref-type="bibr" rid="R43">Montgomery &#x0026; Dennis, 2004</xref>). Det finns ett behov av &#x00F6;kad kunskap om aktiviteter s&#x00E5; som fritidsaktiviteter som icke farmakologisk behandling i m&#x00F6;tet med &#x00E4;ldre som har s&#x00F6;mnbesv&#x00E4;r.</p>
<p>I Sverige &#x00E4;r ca 20 procent av befolkningen 65 &#x00E5;r eller &#x00E4;ldre. Flera studier har visat att ungef&#x00E4;r h&#x00E4;lften av de som &#x00E4;r 65 &#x00E5;r eller &#x00E4;ldre lider av s&#x00F6;mnsv&#x00E5;righeter. Hos &#x00E4;ldre m&#x00E4;nniskor best&#x00E5;r vanligen s&#x00F6;mnbesv&#x00E4;r av det som kallas f&#x00F6;r insomni vilket karakteriseras av att inte sova tillr&#x00E4;ckligt bra eller tillr&#x00E4;ckligt l&#x00E4;nge. Det inkluderar sv&#x00E5;righeter med insomning, ytlig s&#x00F6;mn nattetid, tidigt uppvaknande p&#x00E5; morgonen (<xref ref-type="bibr" rid="R17">Foley et al. 2004</xref>). Dagtid leder detta till energil&#x00F6;shet, tr&#x00F6;tthet, nedst&#x00E4;mdhet, k&#x00E4;nslom&#x00E4;ssiga sv&#x00E4;ngningar, sv&#x00E5;righeter att fokusera och komma ih&#x00E5;g saker. Insomni inneb&#x00E4;r s&#x00E5;ledes ett s&#x00F6;mnproblem d&#x00E4;r symtom kan vara framtr&#x00E4;dande under hela dygnet (<xref ref-type="bibr" rid="R38">Mallon et al., 2014</xref>; <xref ref-type="bibr" rid="R29">Kim et al., 2009</xref>).</p>
<p>Orsaken till s&#x00F6;mnsv&#x00E5;righeter och insomni kan vara livsomst&#x00E4;ndigheter s&#x00E5; som stress, oro, sorg, social situation, oh&#x00E4;lsa och sjukdom. &#x00D6;verg&#x00E5;ng fr&#x00E5;n arbetsliv till pension har visat sig vara en bidragande riskfaktor till d&#x00E5;lig s&#x00F6;mn (<xref ref-type="bibr" rid="R71">van de Straat &#x0026; Bracke, 2015</xref>; <xref ref-type="bibr" rid="R57">SBU, 2010</xref>). Sjukdomar som relateras till bristande s&#x00F6;mn hos &#x00E4;ldre &#x00E4;r bland annat sm&#x00E4;rta, minnesproblem, hj&#x00E4;rtsjukdom och depression (<xref ref-type="bibr" rid="R19">Gooneratne &#x0026; Vitiello, 2014</xref>; <xref ref-type="bibr" rid="R42">Monjan, 2010</xref>; <xref ref-type="bibr" rid="R29">Kim et al., 2009</xref>; <xref ref-type="bibr" rid="R55">Riemann et al., 2015</xref>). Det dagliga livet p&#x00E5;verkas negativt av s&#x00F6;mnbesv&#x00E4;r vilket &#x00E4;ven leder till s&#x00E4;mre livskvalit&#x00E9; (<xref ref-type="bibr" rid="R67">Sun et al., 2022</xref>; <xref ref-type="bibr" rid="R78">Wilson et al., 2019</xref>). Mot bakgrund av n&#x00E4;mnda forskning understryks vikten av att behandla s&#x00F6;mnbesv&#x00E4;r med specifika behandlingsmetoder som &#x00E4;r relevanta f&#x00F6;r de olika symtomen.</p>
<p>L&#x00E4;kemedelsanv&#x00E4;ndning mot s&#x00F6;mnbesv&#x00E4;r &#x00E4;r stor bland &#x00E4;ldre m&#x00E4;nniskor i j&#x00E4;mf&#x00F6;relse med yngre personer trots stor risk f&#x00F6;r biverkningar (<xref ref-type="bibr" rid="R63">Socialstyrelsen 2017</xref>). F&#x00F6;rsiktighet b&#x00F6;r beaktas vid behandling med dessa l&#x00E4;kemedel p&#x00E5; grund av de fysiologiska f&#x00F6;r&#x00E4;ndringarna kopplade till h&#x00F6;g &#x00E5;lder som inneb&#x00E4;r l&#x00E5;ngsam metabolism av l&#x00E4;kemedel. Vanliga biverkningar hos &#x00E4;ldre personer &#x00E4;r exempelvis delirium, &#x00F6;kad reaktionstid och &#x00F6;kad risk f&#x00F6;r fall. Det har ocks&#x00E5; p&#x00E5;visats ett negativt samband mellan anv&#x00E4;ndning av lugnande l&#x00E4;kemedel och fysisk funktion (Parfit et al. 2020). Enligt <xref ref-type="bibr" rid="R63">Socialstyrelsen (2017)</xref> b&#x00F6;r farmakologisk behandling av s&#x00F6;mnsv&#x00E5;righeter endast till&#x00E4;mpas d&#x00E5; individen inte kan eller vill prova en icke-farmakologisk behandling. Relativt enkla &#x00E5;tg&#x00E4;rder som allm&#x00E4;nna r&#x00E5;d, instruktion och utbildning kan bidra till s&#x00F6;mnf&#x00F6;rb&#x00E4;ttringar hos &#x00E4;ldre (<xref ref-type="bibr" rid="R13">Chung et al. 2017</xref>). S&#x00F6;mnsv&#x00E5;righeter kan ocks&#x00E5; behandlas med psykologiska beteendef&#x00F6;r&#x00E4;ndringar och aktiverande syssels&#x00E4;ttning (<xref ref-type="bibr" rid="R44">Morgenthaler et al., 2006</xref>; <xref ref-type="bibr" rid="R18">Gibson et al., 2014</xref>). Icke-farmakologisk behandling innefattar &#x00E4;ven kognitiv beteendeterapi f&#x00F6;r insomni (KBT-I), s&#x00F6;mnhygien, avslappnings&#x00F6;vningar och fysisk aktivitet (<xref ref-type="bibr" rid="R56">Rodriguez et al., 2015</xref>; <xref ref-type="bibr" rid="R49">Okajima, 2017</xref>; <xref ref-type="bibr" rid="R19">Gooneratne &#x0026; Vitiello, 2014</xref>; <xref ref-type="bibr" rid="R43">Montgomery &#x0026; Dennis, 2004</xref>).</p>
<p>Fritidsaktiviteter definieras som n&#x00E5;got en person g&#x00F6;r som inte &#x00E4;r relaterat till arbete och som g&#x00F6;rs p&#x00E5; en viss plats vid en viss tidpunkt. Fritidsaktiviteter kan delas in i aktiv fritid som exempelvis motion, tr&#x00E4;dg&#x00E5;rdsarbete och fysisk aktivitet eller social fritid som till exempel s&#x00E5;ngstund, s&#x00E4;llskapsspel och tr&#x00E4;ffa v&#x00E4;nner (<xref ref-type="bibr" rid="R1">Adams et al., 2011</xref>; <xref ref-type="bibr" rid="R39">Marcum, 2013</xref>; <xref ref-type="bibr" rid="R41">Miyake et al., 2010</xref>; <xref ref-type="bibr" rid="R35">Lee &#x0026; Kim, 2008</xref>). Ordet fritid definieras som den del av dygnet och veckan d&#x00E4;r arbete, s&#x00F6;mn och m&#x00E5;ltider inte &#x00E4;r inkluderade (<xref ref-type="bibr" rid="R46">Nationalencyklopedin, 2023</xref>). Generellt inkluderar fritidsaktivitet som tidigare n&#x00E4;mnts m&#x00E5;nga olika aktiviteter. Det &#x00E4;r sv&#x00E5;rt att kategorisera exakt vilka aktiviteter, eftersom fritidsaktivitet utg&#x00E5;r fr&#x00E5;n individens intresse och den egna fysiska och kognitiva f&#x00F6;rm&#x00E5;gan. Fritidsaktiviteter som valts utifr&#x00E5;n eget intresse ger meningsfullhet i livet. Att utf&#x00F6;ra fritidsaktiviteter tillsammans kan upplevas som ett gynnsamt &#x00E5;ldrande samt mening med livet (<xref ref-type="bibr" rid="R36">Litwin &#x0026; Shiovits-Ezra, 2006</xref>; <xref ref-type="bibr" rid="R1">Adams et al., 2011</xref>). Det finns studier som visar att personer som deltar i olika typer av fritidsaktiviteter har minskad risk f&#x00F6;r kognitiv svikt, de upplever f&#x00F6;rb&#x00E4;ttrad s&#x00F6;mn och ett &#x00F6;kat v&#x00E4;lbefinnande. Att anv&#x00E4;nda fritidsaktivitet som ett icke-farmakologiskt behandlingsalternativ kan d&#x00E4;rf&#x00F6;r utg&#x00F6;ra ett kostnadseffektivt och l&#x00E4;ttillg&#x00E4;ngligt s&#x00E4;tt att behandla s&#x00F6;mnsv&#x00E5;righeter hos &#x00E4;ldre (<xref ref-type="bibr" rid="R53">Richards et al., 2005</xref>; <xref ref-type="bibr" rid="R24">Hellstr&#x00F6;m et al., 2011</xref>).</p>
<p>I takt med &#x00E5;ldrandet minskar den fysiska funktionen som i sin tur p&#x00E5;verkar s&#x00F6;mnen. Att vara aktiv och utf&#x00F6;ra meningsfulla aktiviteter &#x00E4;r viktigt f&#x00F6;r &#x00E4;ldre personers h&#x00E4;lsa och v&#x00E4;lbefinnande. Aktivitet och s&#x00F6;mn &#x00E4;r viktiga komponenter f&#x00F6;r v&#x00E4;lbefinnande under &#x00E5;ldrandet (WHO, 2015). Trots de positiva effekterna av fritidsaktiviteter hos &#x00E4;ldre &#x00E4;r detta fortfarande inte ett prioriterat omr&#x00E5;de inom v&#x00E5;rden f&#x00F6;r att behandla s&#x00F6;mnsv&#x00E5;righeter. Forskning visar att det finns behov av att &#x00F6;ka kunskapen om &#x00E4;ldres s&#x00F6;mn och om aktiviteter f&#x00F6;r att st&#x00F6;dja &#x00E4;ldre i att uppleva tillfredsst&#x00E4;llande natts&#x00F6;mn med hj&#x00E4;lp av icke-farmakologisk behandling (<xref ref-type="bibr" rid="R31">Krishnan &#x0026; Hawranik, 2008</xref>; <xref ref-type="bibr" rid="R3">Aiken et al., 2014</xref>). Kunskapen fr&#x00E5;n denna studie kan ge f&#x00F6;rb&#x00E4;ttrad f&#x00F6;rst&#x00E5;else f&#x00F6;r vilka fritidsaktiviteter som har en positiv inverkan p&#x00E5; s&#x00F6;mnen.</p>
</sec>
<sec id="sec3">
<title>Syfte</title>
<p>Syftet med studien var att beskriva effekten av fritidsaktiviteter f&#x00F6;r att f&#x00F6;rb&#x00E4;ttra s&#x00F6;mn hos &#x00E4;ldre.</p>
</sec>
<sec id="sec4">
<title>Metod</title>
<p>Litteraturstudien genomf&#x00F6;rdes som en systematisk litteratur&#x00F6;versikt (<xref ref-type="bibr" rid="R59">SBU, 2023</xref>). Det internationella begreppet PICO (Population, Intervention, Control, Outcome) &#x00E4;r ett ramverk f&#x00F6;r strukturering av forskningsfr&#x00E5;gor vid systematisk litteratur&#x00F6;versikt (<xref ref-type="bibr" rid="R59">SBU, 2023</xref>). PICO anv&#x00E4;ndes i denna litteraturstudie med f&#x00F6;ljande definition:

<p>Inklusionskriterierna var randomiserade, kontrollerade studier skrivna p&#x00E5; engelska publicerade i vetenskaplig tidskrift mellan &#x00E5;r 2013&#x2013;2023. Endast randomiserade kontrollerade studier (RCT) inkluderades f&#x00F6;r att f&#x00E5; en h&#x00F6;gre evidensstyrka (<xref ref-type="bibr" rid="R59">SBU, 2023</xref>). Studierna skulle unders&#x00F6;ka fritidsaktiviteters p&#x00E5;verkan p&#x00E5; s&#x00F6;mn hos personer i &#x00E5;lder 65 &#x00E5;r och &#x00E4;ldre. Artiklar som unders&#x00F6;kte fritidsaktiviteters p&#x00E5;verkan p&#x00E5; s&#x00F6;mn under en viss tid i samband med livshotande sjukdomar (exempelvis cancer och stroke) exkluderades.</p>
</sec>
<sec id="sec5">
<title>Datainsamling</title>
<p>Litteraturs&#x00F6;kningen genomf&#x00F6;rdes i september 2023 i Cinahl och PubMed. S&#x00F6;kningen genomf&#x00F6;rdes med nyckelord relaterade till sleep, och leisure activities. Ett antal s&#x00F6;kord med olika kombinationer av dem har anv&#x00E4;nts: leisure activities, &#x201D;sleep&#x201D;, sleep disturbance, sleep problems leisures, activity,&#x201D; physical activities&#x201D;, &#x201D;aged&#x201D;, elderly, Totalt identifierades 678 studier via databaserna Cinahl och Pubmed. Av dessa utesl&#x00F6;ts 11 artiklar eftersom det f&#x00F6;rekom dubbletter samt en studie som inte uppfyllde kriterierna f&#x00F6;r publikations&#x00E5;r. De &#x00E5;terst&#x00E5;ende 667 artiklarnas abstrakt granskades, varav 597 exkluderades d&#x00E5; inklusionskriterierna inte uppfylldes. Resterande 70 artiklar l&#x00E4;stes i sin helhet.</p>
<p>Efter att ha l&#x00E4;st den fullst&#x00E4;ndiga texten utesl&#x00F6;ts ytterligare 51 artiklar p&#x00E5; grund av att inklusionskriterierna inte uppfylldes. Slutligen inkluderades 18 studier i litteratur&#x00F6;versikten. S&#x00F6;kstrategin redovisas i ett PRISMA-fl&#x00F6;desschema (se figur 1 i digital appendix).</p>
</sec>
<sec id="sec6">
<title>Analys och Granskning</title>
<p>Samtliga inkluderade artiklar l&#x00E4;stes och analyserades enligt de analyssteg som presenteras i SBU:s granskningsmall &#x201D;Bed&#x00F6;mning av randomiserade studier&#x201D; (<xref ref-type="bibr" rid="R58">SBU, 2020</xref>). De artiklar som inkluderades granskades systematisk och kvalitetsbed&#x00F6;mdes av b&#x00E5;da f&#x00F6;rsta f&#x00F6;rfattarna. Kriterier f&#x00F6;r kvalitetsbed&#x00F6;mning var studiens storlek, bortfallets storlek, reliabilitet och validitet, samt hur v&#x00E4;l metod och urval var beskrivet. Studierna kvalitetsbed&#x00F6;mdes i tre grader: l&#x00E5;g, m&#x00E5;ttlig eller h&#x00F6;g vetenskaplig kvalitet. Samtliga 18 studier var randomiserade kontrollerade studier (RCT). Av dessa saknade fyra studier extern kontrollgrupp, d&#x00E4;r deltagarna var sina egna kontroller. Studierna inkluderades d&#x00E5; de var designade efter RCT och hade en v&#x00E4;l beskriven design, metod och population samt bed&#x00F6;mdes d&#x00E4;rf&#x00F6;r ha m&#x00E5;ttlig till h&#x00F6;g vetenskaplig kvalitet. Artiklarnas beskrivs &#x00F6;versiktligt i en artikel&#x00F6;versikt (se bilaga 1 i digitalt appendix).</p>

<sec id="sec7">
<title>Resultat</title>
<p>Litteratursammanst&#x00E4;llningen baseras p&#x00E5; 18 studier som beskriver effekten av fritidsaktiviteter p&#x00E5; s&#x00F6;mn hos &#x00E4;ldre. I de inkluderade studierna beskrivs olika fritidsaktiviteter som har gett positiv effekt p&#x00E5; s&#x00F6;mnen. Studierna anv&#x00E4;nde subjektiv m&#x00E4;tning med hj&#x00E4;lp av sj&#x00E4;lvskattningsformul&#x00E4;r och/eller objektiv m&#x00E4;tning med hj&#x00E4;lp av aktivitetsklocka. Studiernas respektive m&#x00E4;tmetod redovisas i artikel&#x00F6;versikten (se bilaga 1 i digitalt appendix).</p>
<p>Resultatet presenteras utifr&#x00E5;n f&#x00F6;ljande uppdelning av aktiviteter: Tai Chi/ Hatha Yoga/ Pilates (n=6); Promenader/ Styrketr&#x00E4;ning/ Hemmagympa (n=5); Hush&#x00E5;llsarbete/ Social gruppaktivitet och Fysisk aktivitet (n=2); Vattengympa/ Handmassage (n=2); Dans/ Stretching/ Data-spel (n=3). Studierna genomf&#x00F6;rdes i Asien (n=9), Nordamerika (n=4), Europa (n=2), Sydamerika (n=2), Nordafrika (n=1).</p>
<p>En utf&#x00F6;rligare redovisning av studiernas geografiska ursprung ses i artikel&#x00F6;versikten (Bilaga 1). Urvalsstorlekarna varierande fr&#x00E5;n 10 till 1 635 deltagare i &#x00E5;ldrarna 65&#x2013;97 &#x00E5;r. Interventionernas varaktighet varierade mellan 7 dagar (en studie) &#x2013; 24 m&#x00E5;nader med en frekvens p&#x00E5; 2&#x2013;3 g&#x00E5;nger i veckan upp till en g&#x00E5;ng per dag. Aktiviteternas l&#x00E4;ngd varierade mellan 25&#x2013;60 minuter per tillf&#x00E4;lle. Uppf&#x00F6;ljningstiden efter avslutad intervention varierade mellan 4 dagar och 30 m&#x00E5;nader. En utf&#x00F6;rligare redovisning av urvalsstorlekar och uppf&#x00F6;ljningstid ses i artikel&#x00F6;versikten (se bilaga 1 i digitalt appendix).</p>
</sec>
<sec id="sec8">
<title>Tai Chi/ Hatha yoga/ Pilates</title>
<p>I sex studier utf&#x00F6;rdes interventioner antingen p&#x00E5; &#x00E4;ldreboende eller ordin&#x00E4;rt boende (<xref ref-type="bibr" rid="R61">Siu et al., 2021</xref>; <xref ref-type="bibr" rid="R37">L&#x00FC; et al., 2016</xref>; <xref ref-type="bibr" rid="R9">Chan et al., 2016</xref>; <xref ref-type="bibr" rid="R6">Baklouti et al., 2023</xref>; <xref ref-type="bibr" rid="R21">Hariprasad et al., 2013</xref>; <xref ref-type="bibr" rid="R14">Curi et al., 2018</xref>). <xref ref-type="bibr" rid="R37">L&#x00FC; et al. (2016)</xref> och <xref ref-type="bibr" rid="R9">Chan et al. (2016)</xref> anv&#x00E4;nde Tai Chi; Tai Chi Qigong och Ta Ju Quan involverade mjuka r&#x00F6;relser i samband med lugn andning. Interventionen gav en signifikant subjektiv f&#x00F6;rb&#x00E4;ttring av s&#x00F6;mn. <xref ref-type="bibr" rid="R61">Siu et al. (2021)</xref> j&#x00E4;mf&#x00F6;rde tai chi med tr&#x00E4;ningsprogram best&#x00E5;ende av raska promenader, styrka och aerob tr&#x00E4;ning. En f&#x00F6;rb&#x00E4;ttring av objektiv och subjektiv s&#x00F6;mn (f&#x00E4;rre uppvaknande och mindre vaken tid efter insomnande) kunde ses i b&#x00E5;da interventionsgrupperna. Deltagarna som utf&#x00F6;rde tai chi under interventionens 12 veckor hade en bibeh&#x00E5;llen effekt p&#x00E5; s&#x00F6;mn vid uppf&#x00F6;ljningen efter 24 m&#x00E5;nader. I <xref ref-type="bibr" rid="R6">Baklouti et al. (2023)</xref> utf&#x00F6;rdes ett webbaserat hatha-yoga d&#x00E4;r deltagarna var sj&#x00E4;lvisolerade hemma under Covid-19. <xref ref-type="bibr" rid="R21">Hariprasad et al. (2013)</xref> anv&#x00E4;nde gruppyoga p&#x00E5; &#x00E4;ldreboende. B&#x00E5;da studierna visade p&#x00E5; signifikant f&#x00F6;rb&#x00E4;ttring av den subjektiva s&#x00F6;mnen i j&#x00E4;mf&#x00F6;relse med kontrollgrupp. I studien av <xref ref-type="bibr" rid="R14">Curi et al. (2018)</xref> unders&#x00F6;ktes effekten av pilatestr&#x00E4;ning p&#x00E5; s&#x00F6;mn hos kvinnor. Interventionsgruppen hade en signifikant f&#x00F6;rb&#x00E4;ttring av subjektiv s&#x00F6;mn.</p>
</sec>
<sec id="sec9">
<title>Promenader/ Styrketr&#x00E4;ning/ Hemmagympa</title>
<p>I tre studier utf&#x00F6;rde deltagarna r&#x00F6;rlighet- och balanstr&#x00E4;ning samt styrketr&#x00E4;ning, vilket gav en signifikant f&#x00F6;rb&#x00E4;ttring av subjektiv s&#x00F6;mn (<xref ref-type="bibr" rid="R73">Vaz Fragoso et al., 2015</xref>; <xref ref-type="bibr" rid="R45">Motamedi et al., 2021</xref>; <xref ref-type="bibr" rid="R28">Jimenez-Garcia et al., 2021</xref>). I tv&#x00E5; studier utf&#x00F6;rde deltagarna styrketr&#x00E4;ning i intervaller respektive promenader, vilket gav en f&#x00F6;rb&#x00E4;ttrad objektiv s&#x00F6;mn i j&#x00E4;mf&#x00F6;relse med kontrollgruppen (Zui et al. 2021; <xref ref-type="bibr" rid="R7">Breneman et al. 2019</xref>).</p>
</sec>
<sec id="sec10">
<title>Hush&#x00E5;llsarbete/ Social gruppaktivitet och Fysisk aktivitet</title>
<p><xref ref-type="bibr" rid="R33">Kuck et al. (2014)</xref> utv&#x00E4;rderade effekten av social gruppaktivitet och fysisk aktivitet hos &#x00E4;ldre p&#x00E5; &#x00E4;ldreboende. I det sociala aktivitetsprogrammet ingick minnesfr&#x00E4;mjande &#x00F6;vningar, finmotoriska lekar, kreativa f&#x00E4;rdigheter samt s&#x00E4;llskapsspel och gruppdiskussioner. I den fysiska tr&#x00E4;ningen utf&#x00F6;rde deltagarna &#x00F6;vningar som fr&#x00E4;mjade balans, styrka och uth&#x00E5;llighet med hj&#x00E4;lp av gymnastiksutrustning (t.ex. bollar, hopprep, tennisringar, hantlar och ankelvikter). Samtliga &#x00F6;vningar anpassades utifr&#x00E5;n etablerade fallpreventionsprogram f&#x00F6;r &#x00E4;ldre. Interventions-gruppens subjektiva s&#x00F6;mn f&#x00F6;rb&#x00E4;ttrades signifikant i j&#x00E4;mf&#x00F6;relse med kontrollgruppen. D&#x00E4;remot visade inte de objektiva s&#x00F6;mnparametrarna n&#x00E5;gon skillnad mellan intervention- och kontrollgrupp. Motsatsen p&#x00E5;visades av <xref ref-type="bibr" rid="R60">Seol et al. (2021)</xref> som i sin studie unders&#x00F6;kte friska &#x00E4;ldre kvinnor som &#x00E4;gnade sig &#x00E5;t l&#x00E5;gintensiv fysisk aktivitet (t.ex. promenader) och hush&#x00E5;llsarbete som utf&#x00F6;rdes kv&#x00E4;llstid. B&#x00E5;da dessa aktiviteter gav en f&#x00F6;rb&#x00E4;ttrad objektiv s&#x00F6;mn i j&#x00E4;mf&#x00F6;relse med kontrollgruppen. L&#x00E5;g-intensiv tr&#x00E4;ning gav dessutom en signifikant f&#x00F6;rb&#x00E4;ttring av subjektiv s&#x00F6;mn i j&#x00E4;mf&#x00F6;relse med hush&#x00E5;llsarbete.</p>
</sec>
<sec id="sec11">
<title>Vattengympa/ Handmassage</title>
<p>Deltagarna i studien av <xref ref-type="bibr" rid="R12">Chen et al. (2016)</xref> fick utf&#x00F6;ra vattengympa vilket visade p&#x00E5; en signifikant f&#x00F6;rb&#x00E4;ttring av subjektiv och objektiv s&#x00F6;mn i j&#x00E4;mf&#x00F6;relse med kontrollgruppen. Liknande resultat &#x00E5;terfinns i studien av <xref ref-type="bibr" rid="R32">Kudo et al. (2020)</xref> som anv&#x00E4;nde interventionen handbad och handmassage (utf&#x00F6;rdes i hemmet). Resultatet visade p&#x00E5; f&#x00F6;rb&#x00E4;ttrad subjektiv och objektiv s&#x00F6;mn.</p>
</sec>
<sec id="sec12">
<title>Dans / Stretching/ Dataspel</title>
<p>I studien av <xref ref-type="bibr" rid="R50">Pa et al. (2014)</xref> ingick fyra interventionsgrupper: dansbaserad aerobics och dataspel; dansbaserad aerobics och pedagogisk DVD; stretching och dataspel; stretching och pedagogisk DVD. Dataspel fokuserade p&#x00E5; visuella uppgifter f&#x00F6;r att stimulera minnet, DVD-skivorna inneh&#x00F6;ll pedagogisk f&#x00F6;rel&#x00E4;sning om konst, historia och vetenskap. I samtliga interventioner p&#x00E5;visades signifikant f&#x00F6;rb&#x00E4;ttrad subjektiv s&#x00F6;mn. Liknande resultat &#x00E5;terfanns i studien av <xref ref-type="bibr" rid="R16">Evangelista de Lima et al. (2021)</xref> d&#x00E4;r deltagarna spelade Xboxspel som bestod av olika fysiska aktiviteter anpassade f&#x00F6;r &#x00E4;ldre (utf&#x00F6;rdes individuellt p&#x00E5; dagverksamhet). Resultatet visade att tr&#x00E4;ningen gav en signifikant subjektiv f&#x00F6;rb&#x00E4;ttring av s&#x00F6;mn. I studien av <xref ref-type="bibr" rid="R5">Awick et al. (2017)</xref> utf&#x00F6;rde deltagarna social dans (inspirerad av amerikansk och engelsk folkdans), styrketr&#x00E4;ning och promenader. I alla interventionsgrupper p&#x00E5;visades gynnsam effekt p&#x00E5; subjektiv s&#x00F6;mn.</p>
</sec>
<sec id="sec13">
<title>Diskussion</title>
<p>Syftet med denna studie var att beskriva effekten av fritidsaktivitet f&#x00F6;r att f&#x00F6;rb&#x00E4;ttra s&#x00F6;mn hos &#x00E4;ldre. Det sammantagna forskningsl&#x00E4;get ger st&#x00F6;d f&#x00F6;r att Tai Chi/ Hatha Yoga/ Pilates; Promenad/ Styrketr&#x00E4;ning/ Hemmagympa; Hush&#x00E5;llsarbete/ Fysisk aktivitet och Social gruppaktivitet; Vattengympa/ Handmassage; Dans/ Stretching/ Dataspel har en positiv effekt p&#x00E5; subjektiv och objektiv s&#x00F6;mn hos &#x00E4;ldre. Det &#x00E4;r en v&#x00E4;rdefull kunskap att fritidsaktiviteter kan p&#x00E5;verka b&#x00E5;de de subjektiva och objektiva s&#x00F6;mnparametrarna eftersom s&#x00F6;mn &#x00E4;r ett m&#x00E5;ngdimensionellt fenomen.</p>
<p>Resultatet visade att sociala aktiviteter t.ex. minnesfr&#x00E4;mjande &#x00F6;vningar, finmotoriska lekar, kreativa f&#x00E4;rdigheter samt s&#x00E4;llskapsspel och gruppdiskussioner i kombination med fysisk aktivitet hade en positiv effekt p&#x00E5; den subjektiva s&#x00F6;mnen. I resultatet framkom &#x00E4;ven att dans, dataspel och vattengympa som utf&#x00F6;rdes i grupp ocks&#x00E5; hade positiv effekt p&#x00E5; subjektiv upplevelse av s&#x00F6;mn. Detta st&#x00E4;rks av tidigare studier, d&#x00E4;r <xref ref-type="bibr" rid="R54">Richards et al. (2011)</xref> visade att en kombination av fysisk och social aktivitet leder till signifikant f&#x00F6;rb&#x00E4;ttring av den nattliga s&#x00F6;mnen hos &#x00E4;ldre. Enligt <xref ref-type="bibr" rid="R12">Chen et al. (2016)</xref> har &#x00E4;ldre med st&#x00F6;rre socialt deltagande b&#x00E4;ttre s&#x00F6;mn och generellt f&#x00E4;rre uppvaknande efter insomning. <xref ref-type="bibr" rid="R66">Stehr et al. (2021)</xref> bekr&#x00E4;ftar att &#x00E4;ldre f&#x00F6;redrar att genomf&#x00F6;ra fysiskt aktiva fritidsaktiviteter i grupp d&#x00E5; det visat sig &#x00F6;ka tryggheten n&#x00E4;r n&#x00E5;gon annan leder aktiviteterna. Sammanfattningsvis kan fritidsaktiviteter som genomf&#x00F6;rs i grupp och som best&#x00E5;r av socialt samspel f&#x00F6;rb&#x00E4;ttra &#x00E4;ldres upplevelse av s&#x00F6;mn.</p>
<p>I resultatet framkom ocks&#x00E5; att fritidsaktiviteter som hemmagympa och hush&#x00E5;llsarbete som utf&#x00F6;rts individuellt i hemmamilj&#x00F6; b&#x00E5;da hade en gynnsam effekt p&#x00E5; s&#x00F6;mnen hos &#x00E4;ldre. De som utf&#x00F6;rde hemmagympa rapporterade dock n&#x00E5;got b&#x00E4;ttre subjektiv s&#x00F6;mnupplevelse i j&#x00E4;mf&#x00F6;relse med bara hush&#x00E5;llsarbete. En m&#x00F6;jlig orsak kan vara att tr&#x00E4;ning i sig skapar k&#x00E4;nsla av sj&#x00E4;lvst&#x00E4;ndighet, prestation och tillfredsst&#x00E4;llelse i j&#x00E4;mf&#x00F6;relse med hush&#x00E5;llsarbete som &#x00E4;r f&#x00F6;rknippat med arbetsrelaterat beteende framf&#x00F6;r allt hos &#x00E4;ldre kvinnor (<xref ref-type="bibr" rid="R76">Wong &#x0026; Almeida, 2013</xref>). <xref ref-type="bibr" rid="R80">Zhao et al. (2023)</xref> visar att &#x00E4;ldre som engagerar sig i en specifik fritidsaktivitet som de sj&#x00E4;lva finner mening i leder till &#x00F6;kat v&#x00E4;lbefinnande. Hush&#x00E5;llsarbete har potential att ge intellektuell stimulans och fysisk aktivitet. <xref ref-type="bibr" rid="R24">Hellstr&#x00F6;m et al. (2014)</xref> visar p&#x00E5; liknande resultat d&#x00E4;r underh&#x00E5;ll och reparation av egna hemmet var en syssels&#x00E4;ttning som minskade risken f&#x00F6;r s&#x00F6;mnsv&#x00E5;righeter. Hemmet kan f&#x00F6;r &#x00E4;ldre associeras med autonomi, sj&#x00E4;lvst&#x00E4;ndighet och sociala kontakter. Att delta i meningsfulla aktiviteter i hemmet kan ha d&#x00E4;rf&#x00F6;r en positiv inverkan p&#x00E5; b&#x00E5;da s&#x00F6;mn och autonomi (<xref ref-type="bibr" rid="R20">Haak et al., 2007</xref>; <xref ref-type="bibr" rid="R30">King &#x0026; Farmer, 2009</xref>). Hemmagympa kan &#x00E4;ven vara en billig aktivitet f&#x00F6;r &#x00E4;ldre med ekonomiska begr&#x00E4;nsningar. Tidigare forskning visar att &#x00E4;ldre personer beh&#x00F6;ver hj&#x00E4;lp med att hitta meningsfulla aktiviteter som de har r&#x00E5;d med att utf&#x00F6;ra (<xref ref-type="bibr" rid="R15">Eagers et al., 2016</xref>). Sammantaget kan l&#x00E4;ttillg&#x00E4;ngliga fritidsaktiviteter i hemmamilj&#x00F6; ge &#x00E4;ldre personer k&#x00E4;nslom&#x00E4;ssig och intellektuell tillfredsst&#x00E4;llelse och p&#x00E5; s&#x00E5; vis ha positiva konsekvenser p&#x00E5; s&#x00F6;mnen.</p>
<p>Resultat av denna studie har visat att fysiskt aktiva fritidsaktiviteter (promenader och styrketr&#x00E4;ning anpassade f&#x00F6;r &#x00E4;ldre) med en frekvens p&#x00E5; 2&#x2013;3 g&#x00E5;nger i veckan hade en positiv effekt p&#x00E5; s&#x00F6;mnen. Tidigare forskning visar att &#x00F6;vningar med l&#x00E5;g intensitet f&#x00F6;r &#x00E4;ldre &#x00E4;r att f&#x00F6;redra p&#x00E5; grund av b&#x00E4;ttre f&#x00F6;ljsamhet, l&#x00E4;gre risk f&#x00F6;r skador och b&#x00E4;ttre s&#x00F6;mn (<xref ref-type="bibr" rid="R70">Tse et al., 2015</xref>; <xref ref-type="bibr" rid="R2">Akbari Kamrani, 2014</xref>). Samtidigt visar en studie av <xref ref-type="bibr" rid="R54">Richards et al. (2011)</xref> att h&#x00F6;gintensiv fysisk aktivitet f&#x00F6;rb&#x00E4;ttrar s&#x00F6;mnen hos &#x00E4;ldre. <xref ref-type="bibr" rid="R72">Vanderlinden et al. (2020)</xref> visade att fysisk aktivitet med m&#x00E5;ttlig intensitet 3 g&#x00E5;nger i veckan hade en signifikant effekt p&#x00E5; s&#x00F6;mnresultat hos &#x00E4;ldre. Detta kan tolkas som att &#x00F6;vningar med olika intensitet och olika frekvens samtliga har positiva effekter p&#x00E5; s&#x00F6;mn hos &#x00E4;ldre. <xref ref-type="bibr" rid="R24">Hellstr&#x00F6;m et al. (2015)</xref> belyser vikten av att skapa en v&#x00E5;rdmilj&#x00F6; som ger m&#x00F6;jligheter till dagliga aktiviteter d&#x00E5; det ledde till f&#x00E4;rre tupplurar och bidrog till b&#x00E4;ttre s&#x00F6;mn nattetid. I denna litteratursammanst&#x00E4;llning framkom att anv&#x00E4;ndning av hj&#x00E4;lpmedel p&#x00E5; dagverksamhet i form av Xboxspel med olika aktiviteter bidrog till b&#x00E4;ttre s&#x00F6;mn hos &#x00E4;ldre. Att anv&#x00E4;nda hj&#x00E4;lpmedel i form av spel kan d&#x00E4;rf&#x00F6;r vara implementerbart och l&#x00E4;ttillg&#x00E4;ngligt f&#x00F6;r att &#x00F6;ka aktivitetsniv&#x00E5;n hos &#x00E4;ldre oavsett funktionsf&#x00F6;rm&#x00E5;ga och boendeform.</p>
<p>Resultatet visade att Tai Chi- och Hatha Yoga som utf&#x00F6;rdes p&#x00E5; &#x00E4;ldreboende, dagverksamhet och i hemmet f&#x00F6;rb&#x00E4;ttrade s&#x00F6;mnen. I interventionerna utf&#x00F6;rdes olika typer av Tai Chi och yoga som skiljer sig i r&#x00F6;relser. Flera studier har p&#x00E5;visat att yoga&#x00F6;vningar minskar stress, som ju kan vara en bidragande faktor till s&#x00F6;mnst&#x00F6;rningar (<xref ref-type="bibr" rid="R8">Cetin et al., 2020</xref>; <xref ref-type="bibr" rid="R10">Chen et al., 2010</xref>). Yoga &#x00E4;r &#x00E4;ven associerat med f&#x00F6;rb&#x00E4;ttrat hum&#x00F6;r och sj&#x00E4;lvf&#x00F6;rtroende, minskad depression och &#x00E5;ngest samt upplevelse av b&#x00E5;de mental och fysisk energi (<xref ref-type="bibr" rid="R34">Lavey et al., 2005</xref>; <xref ref-type="bibr" rid="R52">Pilkington et al., 2005</xref>;). Tai Chi &#x00E4;r ocks&#x00E5; en tr&#x00E4;ningsform f&#x00F6;r kropp och sj&#x00E4;l som inneh&#x00E5;ller fysiska, kognitiva, och meditativa komponenter (<xref ref-type="bibr" rid="R27">Irwin et al., 2008</xref>). Sammantaget finns det allts&#x00E5; vetenskapligt st&#x00F6;d f&#x00F6;r att Tai Chi och Yoga inte bara fr&#x00E4;mjar s&#x00F6;mn utan ocks&#x00E5; &#x00E4;r l&#x00E4;tt att individanpassa och kan utf&#x00F6;ras b&#x00E5;de i hemmet och p&#x00E5; &#x00E4;ldreboende.</p>
<p>Resultatet visade ocks&#x00E5; att handmassage har en god effekt p&#x00E5; s&#x00F6;mn hos &#x00E4;ldre. Intressant nog beh&#x00F6;vdes endast handmassage i 20 minuter innan s&#x00E4;ngg&#x00E5;ende vilket kan vara anv&#x00E4;ndbart i klinisk verksamhet. Detta bekr&#x00E4;ftas av <xref ref-type="bibr" rid="R22">Harris et al. (2012)</xref> som visade att tre minuters massage innan s&#x00E4;ngg&#x00E5;ende &#x00F6;kade den totala s&#x00F6;mntiden och s&#x00F6;mneffektiviteten hos &#x00E4;ldre p&#x00E5; &#x00E4;ldreboende. Orsaken till s&#x00F6;mnsv&#x00E5;righeter hos &#x00E4;ldre kan vara sp&#x00E4;nda muskler och nervsp&#x00E4;nningar. <xref ref-type="bibr" rid="R40">Mei et al. (2017)</xref> fann i sin studie att massage hade en tr&#x00F6;stande och avslappnande effekt samt lindrade &#x00E5;ngest. Handmassage som en &#x00E5;tg&#x00E4;rd mot s&#x00F6;mnbesv&#x00E4;r kan bidra till att utveckla en god relation och m&#x00F6;jligg&#x00F6;r f&#x00F6;r v&#x00E5;rdpersonalen att bara vara i stunden tillsammans med den &#x00E4;ldre. Handmassage kan vara l&#x00E4;ttare att utf&#x00F6;ra p&#x00E5; &#x00E4;ldreboende &#x00E4;n p&#x00E5; ordin&#x00E4;rt boende d&#x00E5; det kan vara en personal- och kostnadsfr&#x00E5;ga.</p>
</sec>
<sec id="sec16">
<title>Styrkor och svagheter</title>
<p>Endast randomiserade kontrollerade studier inkluderades eftersom de oftast har en h&#x00F6;g evidensstyrka vilka har f&#x00F6;rst&#x00E4;rkt validiteten i litteratursammanst&#x00E4;llningen (<xref ref-type="bibr" rid="R59">SBU, 2023</xref>). Detta inneb&#x00E4;r att erfarenheter fr&#x00E5;n studier med annan design och annan metod, t.ex. tv&#x00E4;rsnittsstudier eller kvalitativa studier saknas. Kvinnorna var &#x00F6;verrepresenterade i de inkluderade studierna vilket f&#x00F6;ranleder en oj&#x00E4;mn k&#x00F6;nsf&#x00F6;rdelning. Slutsatsen fr&#x00E5;n litteratur&#x00F6;versikten g&#x00E4;ller generellt &#x00E4;ldre utan livshotande sjukdom och d&#x00E4;rf&#x00F6;r b&#x00F6;r slutsatser dras med f&#x00F6;rsiktighet. Det finns ett behov av fortsatt forskning med studier med st&#x00F6;rre deltagarantal, l&#x00E4;ngre uppf&#x00F6;ljning och en j&#x00E4;mnare k&#x00F6;nsf&#x00F6;rdelning. I de inkluderade studierna m&#x00E4;ttes de subjektiva s&#x00F6;mnresultaten med hj&#x00E4;lp av validerade fr&#x00E5;geformul&#x00E4;r och objektiv s&#x00F6;mn genom handledsklocka. I fem studier unders&#x00F6;ktes f&#x00F6;rh&#x00E5;llandet mellan objektiv och subjektiv s&#x00F6;mn hos &#x00E4;ldre och endast tre av dessa visade p&#x00E5; ett samband. Eftersom s&#x00F6;mnrytmen f&#x00F6;r&#x00E4;ndras med &#x00E5;ldern kan det leda till att &#x00E4;ldre anpassar sin uppfattning om s&#x00F6;mnkvalitet till de faktiska f&#x00F6;r&#x00E4;ndringarna i s&#x00F6;mnen samtidigt som de inte kan minnas hur natts&#x00F6;mnen varit tidigare (<xref ref-type="bibr" rid="R23">Harvey et al., 2008</xref>). F&#x00F6;r att f&#x00E5; en &#x00F6;kad f&#x00F6;rst&#x00E5;else f&#x00F6;r fritidsaktiviteters betydelse f&#x00F6;r s&#x00F6;mn beh&#x00F6;vs b&#x00E5;de mer forskning med fokus p&#x00E5; m&#x00E4;nniskors upplevelser av sin s&#x00F6;mnkvalitet, det vill s&#x00E4;ga subjektiva m&#x00E5;tt, men ocks&#x00E5; studier med objektiva m&#x00E5;tt som m&#x00E4;ter s&#x00F6;mnl&#x00E4;ngd och s&#x00F6;mneffektivitet.</p>
</sec>
<sec id="sec14">
<title>Slutsats och kliniska implikationer</title>
<p>Studien visar att olika typer av fritidsaktiviteter kan ses som en h&#x00E4;lsofr&#x00E4;mjande och l&#x00E4;ttillg&#x00E4;nglig icke-farmakologisk &#x00E5;tg&#x00E4;rd mot s&#x00F6;mnbesv&#x00E4;r hos &#x00E4;ldre. Fritidsaktiviteter kan ha en positiv inverkan p&#x00E5; s&#x00F6;mn hos &#x00E4;ldre genom att fr&#x00E4;mja avslappning och fysisk aktivitet. Det kan ocks&#x00E5; skapa sociala interaktioner och stimulans, vilket kan bidra till f&#x00F6;rb&#x00E4;ttrad s&#x00F6;mn.</p>
<p>Studien ger en &#x00F6;kad kunskap och en djupare f&#x00F6;rst&#x00E5;else f&#x00F6;r vilka fritidsaktiviteter som kan p&#x00E5;verka &#x00E4;ldre personers s&#x00F6;mn. Denna kunskap kan hj&#x00E4;lpa sjuksk&#x00F6;terskor och annan v&#x00E5;rdpersonal att i m&#x00F6;tet med den &#x00E4;ldre finna meningsfulla och individanpassade aktiviteter. Att utf&#x00F6;ra aktiviteter som tillfredsst&#x00E4;ller personens k&#x00E4;nslom&#x00E4;ssiga och intellektuella behov &#x00F6;kar i sin tur v&#x00E4;lbefinnandet och har en positiv inverkan p&#x00E5; s&#x00F6;mn. Fritidsaktivitet b&#x00F6;r d&#x00E4;rf&#x00F6;r vara f&#x00F6;rstahandsvalet som en icke-farmakologisk &#x00E5;tg&#x00E4;rd f&#x00F6;r behandling av s&#x00F6;mnbesv&#x00E4;r hos &#x00E4;ldre inom h&#x00E4;lso- och sjukv&#x00E5;rd.</p>
