Choice of primary care in Sweden

Abstract

Through a discourse analysis of the end-users’ statements on their choice of primary care, there is a focus on how they use certain discourses in society with regard to which discourses governs their choices of primary care. For this purpose, a group interview was administered in a location in the south of Sweden. It was strategically designed to on the whole include individuals with following characteristics: age between 20-45 years, and 65 year or older, and also living in a small community. The following main discourses have been identified in the discussion; freedom of choice; i.e. to say that one has actively chosen one’s health centre or doctor, to be able to reject and reselect care-givers, networking; i.e. ‘to say that friends’ and acquaintances’ experiences affect the choice of a new health centre and professional service, i.e. to say that doctors and other staff should give professional service. It seems like choice of care has improved the possibilities of the citizens to choose preferred care provider, or drop one due to dissatisfaction. When implementing reforms in health care it is valuable to take into account the voices of the users, as they are able to contribute to the development of health care.

Lars Nordgren, Department of Service Management, Lund University

Lars Nordgren ec.dr., is Associate Professor in Service Studies and Service Management at Lund University in Sweden. He was the manager of the section of Health Management at the department of Service Management at Lund University 2003-2008. His research interest is in the service sector, especially in healthcare, with an orientation towards managing, marketization processes, and customer orientation in healthcare. A special interest is in health care matching and development of service productivity inspired by the concept of value creation and co-creation. He has written numerous articles and book chapters on these subjects and has done research on behalf of several Official Health Authorities in Sweden.

Bengt Ahgren, Nordic School of Public Health NHV in Gothenburg

Bengt Ahgren MPolSc, PhD, is Professor in public health at the Nordic School of Public Health in Göteborg, Sweden. His research interest is in the creation of integrated health care with an orientation towards management, organisation and evaluation issues. Bengt has among other things studied if competition and integration among care providers are compatible conditions. He has worked for many years as management consultant, e.g. at the Swedish Institute for Health Service Development (Spri) and the Scandinavian Institute of Administrative Research (SIAR), with assignments for the European Commission and several European health care organisations.
